SPECIAL NOTICE: Enrollment and Statement of Health Forms
Enrollment and Statement of Health forms now open as Zip files. After choosing an Enrollment or Statement of Health form, a pop-up window will display. Click on the Download File button and a file download box will appear. Click Open to display the form and a temporary file will be loaded on your computer.
