When downloading forms, your browser may display a security message. Click on the message to view your options and select "Download File..." (see sample image below).

You will be promted to either open the zip file directly or save it to your computer where you can open it at a later time.

Once you open the zip file, you will be able to view all the documents that were in the zip file (see sample image below). Double-click each document to open and print. When printing the enrollment form, make sure to print the CA LAP & Privacy Notices. Make sure to attach these notices to the printed enrollment form.
